Our tax assistance is FREE for
Deaf and HOH low income.
For information on what to bring
with you to your tax appt., please see other side.
Please Note:
If you do not have all of the required
information to file taxes, then your tax appt. will be
rescheduled for a later date.

CHS is an E-File site.
All taxes will be submitted electronically via
computer for faster processing.
Important items to
bring to your tax appointment:
MUST
BRING-We CANNOT do your
taxes if you do not bring this information.
State ID or Driver’s License
Social Security Card or Letter from Social Security with
your name and SS number. (copies are acceptable)
Social security cards for your children, if you are
planning to claim them on taxes.
W-2 Forms from
ALL jobs worked in 2011.
MUST BRING for ALL jobs or will mess up taxes.
Also important to
bring:
Copy of Completed 2010 Tax Papers (We cannot get copies
of your taxes the day of appt)
Bank information or cancelled check or Account number
and routing number (if you want direct deposit of your
refund)
SSA-1099 forms
INT- 1099 forms OR Any papers that say “1099” on top.
2011 Alimony Payments/Receipts
Real estate tax information
(if you own a house or condo and paid house/condo taxes
in 2011)
For house/condo, please bring tax bill with Property
Index Number for IL taxes.
Any papers you receive in the mail that say “Important
Tax Information” on it.
Other Important
Information:
Anyone who wants their taxes done at CHS must come to
the appointment.
We will not do taxes for anyone
NOT
present here at CHS for the appointment.
If filing together, then Husband and Wife must
come to the appointment together.
We will not file separate taxes for your grown up
children unless your children come to the appointment.
These dates are for income tax assistance only.
No Circuit Breaker
forms will be completed on these dates.

Circuit Breaker assistance, please contact Kim E. to
make an appointment.
We do NOT
prepare Schedule E (rental income)forms.
We prepare some limited Schedule C (self employed
or independent contractor) forms.
We do NOT prepare complicated taxes.
